Information you provide

When you submit an intake, we receive your name, role, company, contact details, and business information you choose to provide. Optional questions cover company size, revenue range, industry, systems, and services of interest. Please do not submit passwords, patient records, payment details, or other sensitive personal information.

How we use it

We use your information to review your inquiry, contact you about your needs, prepare a proposed scope, and administer an engagement you agree to. Submitting an inquiry does not purchase Discovery or sign you up for a marketing newsletter.

Where intake information goes

Your intake is submitted to Jotform and stored there for Luxe Intelligence to review. A notification containing your responses and a PDF copy is also delivered to our business email account. Hosting and email providers process information as needed to operate these services. See Jotform’s Privacy Policy for its handling of information on its platform.

Technical information and cookies

Our hosting and form providers may process technical information such as IP addresses, browser information, and request logs to deliver and protect their services. This version of our website does not load the Meta Pixel or advertising analytics. Jotform may use cookies and anti-spam checks when processing your submission; its own policies apply on its pages.

Sharing

We do not sell your personal information. We use service providers to host the website, process submissions, and deliver email. We may also disclose information when required by law. We do not sell or share mobile numbers or SMS consent with third parties or affiliates for marketing purposes.

Text messages

SMS consent is optional and separate from submitting an inquiry. If you opt in, your number may be used to respond about your inquiry. Withdraw consent by replying STOP or contacting us. See our SMS Terms.

Retention and security

We retain inquiry records as needed to respond, manage the business relationship, and meet applicable recordkeeping obligations. Records may exist in both Jotform and our business email. No method of electronic transmission or storage is completely secure.

Your choices

You may leave optional fields blank or email us instead of using the form. To request access, correction, or deletion of your information, contact support@luxeintelligence.co. We may need to verify your identity and retain information where legally required.
